Self-Assessment & Contact Forms
When visitors select the ‘submit ratings’ button and ‘submit to contact John’ buttons, the following form data is saved:
- Data entered by the user:
- Contact Form: First Name, Last Name, Email, Comment or Message
- Self-assessment Form: in the First Name, Last Name, Email, 6 elements for each of the 3 categories for a total of 18 thoughtfulness measures, calculated average rating for each of the 3 categories (3 values in total), and calculated overall thoughtful rating
- Entry Date & Time
- User Agent = the line of text automatically sent by the user’s web browser which typically contains browser, device type, operating system, and browser version
- User IP address
This data is the default data collected by the programs and operating systems.
Cookies
If you leave a comment or submit data in the form’s fields, this information is collected in cookies. These are for your convenience so that you do not have to fill in your details again when you leave another comment or fill out another form.
Embedded content from other websites
Articles on this site may include embedded content (e.g. videos, images, articles, etc.). Embedded content from other websites behaves in the exact same way as if the visitor has visited the other website.
These websites may collect data about you, use cookies, embed additional third-party tracking, and monitor your interaction with that embedded content, including tracking your interaction with the embedded content if you have an account and are logged in to that website.

The Leaders Coach does not share or process any data from the forms.
How long we retain your data
If you submit a contact or self-assessment form, the data you entered and its metadata are retained indefinitely. This is so we can have your contact information if we have to contact you and your self-assessment results if you request a copy or we need to refer to them in the future.
What rights you have over your data
If you have submitted a contact or self-assessment form, you can request to receive an exported file of the form data we hold about you, including any data you have provided to us. You can also request that we erase any contact or self-assessment data we hold about you. This does not include any data we are obliged to keep for administrative, legal, or security purposes.
